They’ll work with you, your family, and your physician to create a customized care plan that fits your needs. There are many care options, whether you need assistance for a few hours at a time or are looking for a live-in caregiver.</p> <p>Home health aides and certified nursing assistants can help you with activities of daily living, such as bathing, toileting, dressing, transfers, and feeding. They’ll also gladly assist you around the house by preparing nutritious meals, doing light housework, escorting you to appointments, running errands, and going shopping for you.</p> <p>Licensed practical nurses and registered nurses can provide care ordered by your doctor. This includes medication management, wound care, and other skilled care. You’ll also work with a nurse as your dedicated case manager, who will attend to all of your assessments to make sure your care plan is satisfactory.
